GGNet is centralizing intensive clinical care in a new facility, The Spreng. Four departments will move into the new building on GGNet's main campus in Warnsveld. Collaboration between the four departments creates opportunities to provide tailored and future-proof care. Panton has been asked to identify these opportunities and shape the collaboration.

Approach
Co-creation was used to map out the patient journey. This process identified opportunities for improved collaboration and better care. The following opportunities were explored further through multiple iterations::
- More person-centered care
- Prevention, avoiding (readmissions)
- Seamless alignment between departments
- Sharing expertise

Result
The outcome is a shared vision: Working together to ensure perspective and continuity.
The vision focuses on strengthening collaboration at three levels:
- at the HIKZ
- with patients and their loved ones
- with outpatient and other clinics
Panton supports GGNet with the implementation of this vision.
